Annual Reviews of Long-Term Conditions

As from the 1^st April 2025 we are introducing a new system for inviting patients to attend their annual review.

All patients on a long term condition register will be invited for their annual review in the month of their birthday, you will be sent an electronic invite to self book in the appropriate clinic.

This may mean that some patients who have recently attended for their annual review and have a birthday in April will be reinvited, this will only happen for the first year of running the new system.  I.e. attended in December, January, February or March and have a birthday in April will be reinvited in April 2025 then not again until April 26.
