CQC is the independent regulator of all health and social care in England. They monitor, inspect and regulate all hospitals, care homes, home-care agencies, Gp practices and dental practices.

General Data Protection Regulation
The GDPR is a new law that determines how your personal data is processed and kept safe, and the legal rights that you have in relation to your own data.
The regulation applies from 25 May 2018, and will apply even after the UK leaves the EU.

Information Technology Statement of Intent
STATEMENT OF INTENT FOR GOODHEART SURGERY
New contractual requirements came into force from 1 April 2014 requiring that GP Practices should make available a statement of intent in relation to the following Information Technology (IT) developments:
- Summary Care Record (SCR)
- GP to GP Record Transfers
- Patient Online Access to Their GP Record
- Data for commissioning and other secondary care purposes
The same contractual obligations require that GP Practices have a statement of intent regarding these developments in place and publicised by 30 September 2014.
Summary Care Record (SCR)
NHS England requires practices to enable successful automated uploads of any changes to patient’s summary information, at least on a daily basis, to the summary care record (SCR), or have published plans in place.
Having your Summary Care Record (SCR) available will help anyone treating you without your full medical record. They will have access to information about any medication you may be taking and any drugs that you have a recorded allergy or sensitivity to.
Of course if you do not want your medical records to be available in this way then you will need to let us know so that we can update your record. You can do this via an opt-out form which is available at the practice reception.
Your summary care record is automatically updated, on at least a daily basis, to ensure your information is correct.
GP to GP Record Transfers
NHS England requires practices to utilise the GP2GP facility for the transfer of patient records between practices, when a patient registers or de-registers (not for temporary registration).
It is very important that you are registered with a doctor at all times. If you leave your GP and register with a new GP, your medical records will be removed from your previous doctor and forwarded on to your new GP via NHS England.
It can take your paper records up to two weeks to reach your new surgery. With GP to GP Record Transfers your electronic record is transferred to your new practice much sooner. Dr Koshy confirms GP to GP transfers are already activated to send and receive patient records via this system.
Patient Online Access to Their GP Record
NHS England require practices to promote and offer the facility to enable patients to have online access to appointments, prescriptions, allergies and adverse reactions or have published plans in place.
We currently offer the facility for booking and cancelling appointments and also for ordering your repeat prescriptions on-line.
This is done by our clinical system. If you do not already have a user name and password for this system please register at the practice reception.
Data for commissioning and other secondary care purposes
It is already a requirement of the Health and Social Care Act that practices must meet the reasonable data requirements of commissioners and other health and social care organisations through appropriate and safe data sharing for secondary uses, as specified in the technical specification for care data.
Dr Koshy has specific arrangements in place to allow patients to “opt out” of care data which allows for the removal of data from the practice. Please see the information about care data at the practice within the reception / waiting room area.
Dr Koshy confirms these arrangements are in place and that we undertake annual training and audits to ensure that all our data is handled correctly and safely via the Information Governance Toolkit.
Care Data
Care data is a forthcoming extract of patient data commisioned by NHS England.
NHS England is commisioning a modern data service from the the Health & Social Care Information Centre (HSCIC) on behalf of the entire health & social care system known as care.data. GPES will be used to extract GP data each month. The identifiers to be extracted are: NHS Number, DOB, Postcode & Gender, which will allow patients’ GP data to be linked to their hospital data. No free text will be extracted, only coded information about referrals, NHS prescriptions and other clinical data.
The confidentiality and security of patient data is of paramount importance which is why GPES has established Information Governance Principles. The intention of the scheme is to make increased use of the information in medical records in order to improve the quality of care for patients.

Other Information
Named Accountable GP
From April 2014 all patients aged 75 years and over and those on the practice ‘Avoiding unplanned admissions’ register were allocated a named accountable GP.
From April 2015 ALL patients have been allocated a named accountable GP. This will be the same as the GP you are currently registered with unless otherwise notified.
This will not affect your right to see the GP of your choice.

Patient Online Access to Their GP Record
NHS England require practices to promote and offer an electronic appointment booking system and online booking of prescriptions facility to enable patients online access to book appointments, view, amend, cancel and print appointments, order prescriptions, view and print a list of their repeat prescriptions for necessary drugs, medicines or appliance. We currently offer this facility via Emis Web, If you do not already have a user name and password for this facility – please contact the practice to register.
NHS England require practices to promote and offer the facility for patients to view online, export or print any summary information from their records i.e. medications, allergies, adverse reactions and any additional information agreed between the GP and patient. The practice now offers this facility, again, you can apply for a user name and password from the practice.
Your NHS Number
The practice includes your NHS number as the primary identifier in all NHS clinical correspondence issued by the practice whether electronic for or otherwise, except in exceptional circumstances where the number cannot be ascertained e.g. when the patient is seen urgently at home.
Care Data

Access to Health Records
In accordance with the Data Protection Act 1984 and the Access to Medical Records Act 1990 patients may request to see their medical records. A charge may be levied for this service as specified by the Data Protection Act and any request for access should be made in writing. Access will be by appointment only. The doctor is entitled to withhold any information that he or she considers could be detrimental to you.
