Named Accountable GP


From April 2014 all patients aged 75 years and over and those on the practice 'Avoiding unplanned admissions' register were allocated a named accountable GP.

From April 2015 ALL patients have been allocated a named accountable GP. This will be the same as the GP you are currently registered with unless otherwise notified.

This will not affect your right to see the GP of your choice.

 

GP2GP Record Transfers


NHS England requires practices to utilise the GP2GP facility for the transfer of patient records between practices, when a patient registers or de-registers (not for temporary registration).

It is very important that you are registered with a doctor at all times. If you leave your GP and register with a new GP, your medical records will be removed from your previous doctor and forwarded on to your new GP via NHS England. It can take your paper records up to two weeks to reach your new surgery. With GP to GP record transfers your electronic record is transferred to your new practice much sooner.

Patient Online Access to Their GP Record


NHS England require practices to promote and offer an electronic appointment booking system and online booking of prescriptions facility to enable patients online access to book appointments, view, amend, cancel and print appointments, order prescriptions, view and print a list of their repeat prescriptions for necessary drugs, medicines or appliance. We currently offer this facility via Emis Web,  If you do not already have a user name and password for this facility – please contact the practice to register.

 

NHS England require practices to promote and offer the facility for patients to view online, export or print any summary information from their records i.e. medications, allergies, adverse reactions and any additional information agreed between the GP and patient. The practice now offers this facility,  again, you can apply for a user name and password from the practice.

 

Your NHS Number


The practice includes your NHS number as the primary identifier in all NHS clinical correspondence issued by the practice whether electronic for or otherwise, except in exceptional circumstances where the number cannot be ascertained e.g. when the patient is seen urgently at home.

 

Care Data


It is a requirement of the Health and Social Care Act that practices must meet the reasonable data requirements of commissioners and other health and social care organisations through appropriate and safe data sharing for secondary uses, as specified in the technical specification for care data.

 

We have specific arrangements in place to allow patients to "opt out" of care.data which allows for the removal of data from the practice. Please contact the practice for further information about care.data.

We undertake annual training and audits to ensure that all our data is handled correctly and safely via the Information Governance Toolkit.

 

Access to Health Records


In accordance with the Data Protection Act 1984 and the Access to Medical Records Act 1990 patients may request to see their medical records. A charge may be levied for this service as specified by the Data Protection Act and any request for access should be made in writing. Access will be by appointment only. The doctor is entitled to withhold any information that he or she considers could be detrimental to you.
